Lost/Damaged Ballots

If you damage, incorrectly mark (spoil), lose, or never receive your mail ballot, a replacement ballot can be issued. A voter with an illness or disability who can’t come to our office may designate an individual to pick up and return a replacement ballot on your behalf.

Lost or Never Received Ballots

  • Allow 10 days from the date the ballot is mailed before reporting a lost ballot. Track your ballot
  • Call or email to request a replacement ballot be issued. Include your full legal name and the address where you want the ballot sent for confirmation.
  • A new ballot will be sent with a Gold Form that needs to be completed and returned with the reissued ballot. The form requires you to affirm that you either lost, spoiled, or did not receive your ballot.
  • You can also vote in person at an Early Voting Location or at your assigned polling place on Election Day.
    • Early Voting:  You will sign a form stating your ballot is lost. You will vote a regular ballot.
    • Election Day:  You will cast a Provisional Ballot. This ballot won’t be counted on Election Day. Once we verify we didn’t receive your mail ballot, we’ll accept and count your Provisional Ballot.

Damaged or Spoiled Ballots

  • You must first return the damaged or spoiled ballot. You can return it via mail or in person, either at an early voting location or at your precinct on Election Day.
  • A new ballot will be sent with a Gold Form that needs to be completed and returned with the reissued ballot. The form requires you affirm that you either lost, spoiled, or did not receive your ballot.
